Job summary

Activate Care seeks a field-based Registered Nurse to travel throughout Massachusetts performing in-person health assessments for dually eligible members in a managed care program. You will document findings in GuidingCare and hand off to care management, with focus on assessment and reporting rather than ongoing case management.

The role requires MA RN licensure, an Associate Degree in Nursing (BSN preferred), a valid driver’s license, and reliable vehicle.

Qualifications

• Active Massachusetts RN license (or higher) in good standing.
• Accredited Associate Degree in Nursing; BSN preferred.
• Valid driver’s license, reliable vehicle, and active auto insurance required.

Responsibilities

• Conduct in-person health assessments in members' homes across MA.
• Document assessment findings in health plan platform per workflows and send SOAP notes.
• Submit documentation within 48 hours of each assessment and address audit findings promptly.
• Follow field-safety guidelines during home visits and report safety concerns.
• Adhere to HIPAA, NCQA standards, and complete mandatory trainings.
