Activate Care is seeking a field-based Registered Nurse to travel across Massachusetts for in-person health assessments of dually eligible members. The role focuses on timely documentation and collaboration with care management teams.

This position offers full-time opportunities with a sign-on bonus and comprehensive benefits. Ideal candidates hold an active MA RN license, an associate degree in nursing (BSN preferred), and at least 1 year of home health experience.

Qualificações

• Active Massachusetts RN license in good standing.
• Associate Degree in Nursing required; BSN preferred.
• Valid driver’s license and reliable vehicle required.
• 1+ year of home health experience required.
• Experience with dual-eligible, Medicaid, or Medicare populations preferred.
• Strong organizational and decision-making skills.

Responsabilidades

• Conduct in-person health assessments in members' homes across Massachusetts.
• Document assessment findings accurately in care management platform.
• Submit documentation within 48 hours of each assessment.
• Follow safety guidelines and mandated reporting standards.
• Maintain HIPAA compliance and complete required trainings.
• Perform other duties as assigned.
