Job summary

CVS Health Corporation is seeking a compassionate and experienced Registered Nurse Case Manager to support members in Essex County and nearby areas. This remote role includes up to 75% field travel and a schedule of 8:00 AM-5:00 PM EST, Monday through Friday.

You will develop individualized care plans, coordinate medical, behavioral and social services, and mentor new team members while ensuring safe transitions of care and timely documentation in the electronic health record.

Qualifications

• Active, unrestricted RN license in New Jersey.
• 3+ years of clinical practice experience.
• 3+ years of home health, hospice, or similar setting experience.
• Residency in Essex County NJ.
• NJ Choice Certification requirements (80% exam, state training modules, field mentoring).
• Willingness to travel up to 75% with mileage reimbursement.

Responsibilities

• Conduct in-home, face-to-face assessments for members enrolled in MLTSS and D-SNP/FIDE.
• Evaluate member needs and coordinate services (adult/pediatric medical day care, personal care, nursing facility placement, MLTSS enrollment).
• Partner with members, caregivers, PCPs, and teams to develop and implement individualized care plans.
• Authorize services per program guidelines and support safe transitions of care.
• Document all case activity accurately and promptly in the EHR.
• Apply clinical judgment to manage complex care needs.
• Mentor new team members as proficiency is achieved.
