Somatus in Seattle, WA is seeking a Field-Based Nurse Case Manager to join our interdisciplinary care team. You will outreach, schedule member visits in homes, dialysis clinics, and physician offices to support high-risk members with barrier assessments and care plan education.

In this role, you will build trusting relationships with nephrology practices, conduct in-home and in-facility visits, and participate in care planning.

Qualifications

• Current, unrestricted RN license in Washington.
• Ability to obtain licensure in other states as needed.
• 3+ years of RN experience.
• Renal, CKD, or dialysis care experience as main focus.
• Computer proficiency with MS Office and Windows navigation.
• Residence with reliable high-speed internet access.

Responsibilities

• Provide complete continuum of quality care via in-person or phone interactions.
• Travel for in-home and in-facility visits with members.
• Collaborate with interdisciplinary team and nephrology practices.
• Document findings and maintain electronic patient records.
• Identify barriers to treatment and improve member engagement.
• Complete safety trainings and maintain field-based competencies.
