Somatus in Seattle is seeking a Field Based Nurse Case Manager to outreach and schedule member face-to-face visits in homes, dialysis clinics, and physician offices to support higher risk members who need barrier assessments and face-to-face care plan education.

Our field nurses build trusting relationships with nephrology practices and collaborate to improve health outcomes. This role involves in-home and in-facility visits, with about 75% field time and strong documentation.

Qualifications

• Current, unrestricted RN license in the state of Washington.
• 3+ years of RN experience, with renal, CKD or dialysis care focus.
• BSN or higher degree; CCM certification preferred.
• Field-based role requiring in-home visits and telephonic management.
• Experience in a managed care environment is a plus.

Responsibilities

• Outreach and schedule member face-to-face visits in homes, dialysis clinics, and physician offices.
• Support high-risk members with barrier assessments and care-plan education.
• Develop trusting relationships with nephrology practices to improve health outcomes.
• Perform in-home and in-facility visits 75% of the time and complete care planning.
• Document findings and maintain safety trainings; navigate electronic systems.
