Somatus is seeking a Field Based Nurse Case Manager to visit members in their homes and clinics, with a focus on renal and chronic kidney disease care. You will lead assessments, develop plans, and coordinate with nephrology practices while traveling extensively in the greater Queens, NY area.

The role requires an unrestricted RN license (US), 3+ years of RN experience, strong computer skills, and field-based experience delivering in-home care.

Qualifications

• Current, unrestricted RN license in the state of New York or ability to obtain licensing in other states
• Strong computer proficiency, MS Office suite, and data entry
• Experience in renal, CKD, or dialysis care
• Field-based in-home visits and engagement with nephrology practices

Responsabilités

• Provide in-home and in-facility visits to high-risk members
• Coordinate care planning with interdisciplinary teams
• Educate patients on conditions and medications; perform medication reviews
• Document findings and maintain records with accuracy
• Travel to communities to support care plans and reduce readmissions
• Establish trusting relationships with members and caregivers
