Job summary

Somatus is seeking Field Based Nurse Case Managers to join our interdisciplinary care team in the Seattle area. You will outreach, schedule member visits in homes, dialysis clinics, and physician offices to support high-risk members, conduct barrier assessments, and educate on care plans to improve health outcomes.

This role involves extensive field work, with 75% travel in the greater Seattle region. Requirements include a current, unrestricted RN license in Washington, 3+ years of RN

Qualifications

• Current, unrestricted RN license in the state of Washington.
• 3+ years of RN experience.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
• Proficiency with MS Office (Word, Excel, PowerPoint and Outlook) and ability to type while navigating Windows.

Responsibilities

• Provide complete continuum of quality care through close communication with members via in-person or on-phone interaction.
• Travel to complete in home and in facility visits with members.
• Travel to participate in care planning in facilities & provider offices.
• Document findings and maintain field-based safety trainings.
