Job summary

Optum Home & Community Care is seeking an RN Field Case Manager to work in home-based settings across Seattle, WA. In this field-based role, you will assess health status in patients' homes, identify barriers to treatment, provide education, and coordinate referrals to support services.

Qualified candidates hold an active WA RN license, BLS, and 1+ year of clinical or home-care experience. A valid driver's license and transportation are required; a BSN is preferred.

Qualifications

• Current, unrestricted RN license in the state of Washington.
• Certified in Basic Life Support, or ability to obtain by start date.
• 1+ years of clinical experience.
• Experience in home care/home visits.
• Reside in or near Seattle, WA.
• Driver's license and access to reliable transportation.

Responsibilities

• Visit members in their homes to assess their current health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to assist with self-management.
• Make referrals to outside sources.
• Provide a complete continuum of quality care through close communication with members via in-person in home or on-phone interaction.
• Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels.
