Job summary

Somatus is hiring a Field Based Nurse Case Manager in the greater Seattle area. You will outreaching and scheduling member visits in homes, dialysis clinics, and physician offices to support higher risk members needing barrier assessments and care plan education.

Qualifications

• Current, unrestricted RN license in the state of Washington
• Ability to get licensed in other states as needed
• 3+ years of RN experience
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job
• Computer proficiency, to include strong data entry, MS Office, and the ability to type while navigating a Windows environment

Responsibilities

• Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
• Travel to complete in home and in facility visits with members
• Travel to participate in care planning in facilities & provider offices
• Use nursing assessment skills to identify medical, behavioral, social determinants of health gaps or barriers
• Provide condition education, conduct medication reviews, assist with advance planning and connect to community resources
• Demonstrate ability to establish trusting relationships with members and caregivers and identify strategies to improve engagement
• Complete in-patient & post-discharge visits to ensure appropriate services are provided
• Document findings; solid computer and software navigation skills are critical
• Complete and maintain field-based safety trainings

Field Based Nurse Case Managers are an important part of the interdisciplinary care team. They are responsible for outreaching and scheduling member face-to-face visits in homes, dialysis clinics, and or physician offices to support higher risk members needing barrier assessments and face-to-face care plan education. In addition, our field nurses develop trusting relationships with the nephrology practices in their markets and work closely with those practices to support improvement in members health outcomes.

This is a field-based position meeting with members in their homes and facilities and will be in the field at least 75% of the time. The territory will primarily cover the greater Seattle area.

• Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
• Travel to complete in home and in facility visits with members
• Travel to participate in care planning in facilities & provider offices
• Use nursing assessment skills to identify medical, behavioral, social determinates of health gaps or barriers in treatment plan
• Provide condition education, conduct medication reviews, assist with advance planning and connect to community resources
• Demonstrate ability to establish trusting relationships with members & caregivers and identify innovative strategies to improve and maintain engagement
• Participate in interactions with interdisciplinary team, treating providers and community resources
• Complete in- patient & post-discharge visits to ensure that discharged members receive the necessary services and resources to focus on reducing readmissions
• Documenting and track findings; solid computer and software navigation skills are critical
• Complete and maintain field based safety trainings

Qualifications:

• Current, unrestricted RN license in the state of Washington
• Ability to get licensed in other states as needed
• 3+ years of RN experience
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job
• Computer proficiency, to include strong data entry, utilizing MS Office (Word, Excel, PowerPoint and Outlook), and the ability to type and talk at the same time while navigating a Windows environment
• Reside in a location that can receive a high speed internet connection or can leverage existing high-speed internet service

Preferred Qualifications:

• BSN or higher level of education
• Diabetic Educator experience
• Field-based experience going into homes
• Telephonic case management experience
• Experience working within a managed care environment

Compensation:

$78,000 to $96,500 per year

Please note only candidates who live in Seattle, WA or that are within a 10 - 20 mile radius of Seattle are the only candidates eligible for the top end this range.
