Field Based Nurse Case Managers are an important part of the interdisciplinary care team. They are responsible for outreaching and scheduling member face-to-face visits in homes, dialysis clinics, and or physician offices to support higher risk members needing barrier assessments and face-to-face care plan education. In addition, our field nurses develop trusting relationships with the nephrology practices in their markets and work closely with those practices to support improvement in members health outcomes.
This is a field-based position meeting with members in their homes and facilities and will be in the field at least 75% of the time. The territory will primarily cover the greater Seattle area.
$78,000 to $96,500 per year
Please note only candidates who live in Seattle, WA or that are within a 10 - 20 mile radius of Seattle are the only candidates eligible for the top end this range.
