Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager to coordinate care for members with complex needs in Hudson County and surrounding areas. You’ll travel regularly to assess and support patients in their homes, with a flexible option to work from home when not in the field.

The role emphasizes independent work, strong clinical judgment, and collaboration with interdisciplinary teams to improve safety and quality of life for community members.

Qualifications

• Active, unrestricted RN license in New Jersey.
• At least 3 years of clinical experience in hospital, home health, or ambulatory care.
• Residence in Hudson County or nearby communities.
• Comfort with frequent local travel and in-home visits.
• NJ Choice Certification required with exam score and state training modules.
• Ability to work independently with a flexible daily schedule.

Responsibilities

• Conduct in-home assessments for MLTSS program members.
• Evaluate medical, behavioral, and social needs to support appropriate care.
• Collaborate with providers, caregivers, and teams for coordinated care.
• Develop and manage individualized care plans to promote safety and quality of life.
• Participate in care team meetings and advocate for member needs.
• Document assessments and care activities in the electronic health record.
• Support care transitions and connect members with community resources.
• Mentor new team members as you grow in the role.
