Qualifications

Active, unrestricted RN license in New Jersey.
Minimum 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings.
Residence in Morris County or nearby communities.
NJ Choice Certification with minimum exam score 80% and state training modules.
Ability to work independently with a flexible daily schedule.

Responsibilities

Conduct in-home assessments for members enrolled in MLTTSS programs.
Evaluate medical, behavioral, and social needs to support appropriate care and services.
Collaborate with providers, caregivers, and interdisciplinary teams to ensure coordinated, holistic care.
Develop and manage individualized care plans that promote safety, independence, and quality of life.
Participate in care team meetings and advocate for member needs.
Document all assessments and care activities accurately in the EHR.
Support members through care transitions and connect them with community resources.
Mentor new team members as you grow in the role.
