Job summary

CVS Health Corporation is seeking a Registered Nurse Case Manager – Field in New Jersey. This field-based role requires travel up to 75% and offers home-office flexibility when not in the field. You will coordinate care for members with complex needs, conducting in-home assessments and collaborating with interdisciplinary teams.

Requirements include an active NJ RN license, 3+ years clinical experience, and NJ Choice Certification. Associate degree in Nursing is required; BSN preferred.

Qualifications

• Active, unrestricted RN license in New Jersey or eligible.
• Minimum 3 years clinical experience in hospital/home health/ambulatory care.
• Residence in Morris County or nearby communities.
• NJ Choice Certification with required components.

Responsibilities

• Conduct in-home assessments for members enrolled in MLTSS programs.
• Evaluate medical, behavioral, and social needs to plan care.
• Collaborate with providers and care teams for coordinated care.
• Develop and manage individualized care plans for safety and quality of life.
• Document assessments and activities in the EHR system.
• Support care transitions and connect with community resources.
