Job summary

Molina Healthcare is seeking care coordinators to support care management across the continuum. You will assess members, develop individualized care plans, and coordinate services with a multidisciplinary team to improve outcomes and reduce costs.

Qualifications

• At least 2 years in health care, care management or related field.
• Licensure/certification required only if state contract mandates it.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and available services.
• Ability to work independently with minimal supervision.
• Strong time management and prioritization skills.

Responsibilities

• Complete member assessments within regulated timelines and identify care coordination candidates.
• Develop and implement care plans with members and care networks.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and document care plan effectiveness and adjust as needed.
• Maintain ongoing member caseload and outreach for management.
• Coordinate integration of behavioral health, LTSS, and community resources.
