Job summary

Somatus in Baton Rouge, LA is seeking an RN Care Manager to support high-need CKD/ESKD patients through in-home visits, clinic coordination, and care planning.

You will collaborate with nephrologists, PCPs, and interdisciplinary teams, delivering education and coordinating transitions of care to improve patient outcomes.

This field-based role requires travel up to 75% and a valid RN license with BLS certification; candidates should be committed to patient-centered care.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years RN experience in multidisciplinary teams with Physicians.
• Valid BLS certification ONLY from a licensed AHA or American Red Cross training facility or provider.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job
• Reside in a location that can receive a high-speed internet connection or can leverage existing high-speed internet service
• Comfortable traveling to partner hospitals, clinics, and community-based facilities within your assigned region to support care coordination, build relationships, and collaborate with clinical teams.
• BSN or higher education preferred
• Field-based experience going into homes (preferred)
• Telephonic case management experience (preferred)

Responsibilities

• Perform home visits and care planning for high‑needs patients.
• Coordinate with nephrologists, PCPs, and care teams.
• Educate patients on CKD/ESKD management and self-care.
• Develop individualized care plans and monitor progress.
• Engage with partner practices to improve outcomes.
