Job summary

Somatus is seeking an RN Care Manager to lead high‑needs CKD/ESKD patient care across home, clinic, and physician office settings. You’ll partner with nephrology teams and health plans to coordinate interdisciplinary care and remove barriers to optimize outcomes.

The role emphasizes direct patient engagement, education, and care planning, with significant field travel within your designated region. A current RN license and BLS are required.

Qualifications

• RN license required and willingness to obtain licensure in other states as needed.
• Two-plus years of RN experience, including multidisciplinary team work.
• BLS certification from an approved provider is required.
• Renal, CKD or dialysis exposure preferred.
• Reliable high-speed internet access at home or in a location with internet.
• Ability to travel to partner hospitals, clinics, and facilities within the assigned region.

Responsibilities

• Conduct home visits, dialysis clinics, and physician offices to support high‑risk patients.
• Coordinate care planning with nephrologists, PCPs, and multidisciplinary teams.
• Educate patients and families on treatment plans and self‑management.
• Develop individualized care plans to address barriers to care and improve outcomes.
• Build trusted relationships with practices and providers to optimize workflows.
• Participate in regular interdisciplinary care team meetings and care coordination activities.
• Travel up to 75% of the time to fulfill duties.
