Job summary

Somatus in the United States is seeking an RN Care Manager to focus on high-needs CKD and ESKD populations, collaborating with physicians, nurses and patient health advocates to improve outcomes.

You will travel to member homes, dialysis clinics, and offices, support care planning, educate patients, and promote coordinated, interdisciplinary care in a physician practice setting.

Qualified candidates have an RN license, 2+ years experience, BLS, and are willing to travel; BSN and CCM are preferred.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in a multi-disciplinary team and physicians.
• Valid BLS certification from an AHA or Red Cross facility.
• Renal, CKD or Dialysis care experience as a main focus of your job.
• Willingness to travel within your assigned region.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Support care team planning, coordinating, and interdisciplinary meetings.
• Develop relationships with patients and provider offices in-person and by telehealth.
• Collaborate with provider practices to optimize workflows and care plans.
• Educate provider practices on the program and care coordination processes.
• Provide complete quality care through ongoing communication and reassessments.
• Travel to member homes, facilities, and offices for visits and care planning.
