Job summary

Elevance Health seeks an LTSS Service Coordinator–RN Clinician to manage members’ care plans across the continuum, performing assessments and coordinating services. This field-based role emphasizes person-centered care and collaboration with health teams to optimize outcomes.

You will develop individualized care plans, set short/long term goals with members and caregivers, and facilitate referrals while navigating benefits and waivers as needed.150+

Qualifications

• Requires a high school diploma or GED equivalent and minimum of 3 years of experience with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in Ohio state(s) required.
• BA/BS in Health/Nursing preferred.
• Strong preference for case management experience with older adults or individuals with disabilities.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify, evaluate, coordinate and manage member needs across physical health, behavioral health, social services and LTSS.
• Manage members with chronic illnesses and co‑morbidities to ensure cost effective utilization of health benefits.
• Obtain thorough member history to develop an individual care plan.
• Establish short and long term goals with the member and caregivers; identify needs for alternative level of care or waiver programs.
• Develop and oversee the member's care plan and ensure access to services.
• Assist with implementation of care plans by facilitating authorizations/referrals within benefits or through extra‑contractual arrangements.
• Interface with Medical Directors and Inter‑Disciplinary Teams on care management.

Education

High School Diploma or GED

Registered Nurse (RN) License in Ohio
