Job summary

Ohio's Hospice is seeking an RN Admission Care Liaison for the South Region. The role involves completing hospice admissions, assessing patient needs, coordinating services, and educating patients and families on hospice options.

The ideal candidate will have strong clinical judgment, excellent communication, and a genuine passion for hospice and palliative care, with willingness to travel throughout the Greater Miami Valley area.

Qualifications

• Graduate of an accredited nursing program.
• Active, unrestricted RN license in Ohio.
• Current CPR certification.
• Strong computer skills and proficiency with EMR.
• Valid driver’s license and reliable transportation.
• Able to travel throughout service area.
• Background check and drug screening completed.

Responsibilities

• Complete hospice admissions accurately and compassionately.
• Perform comprehensive patient assessments and determine services.
• Educate patients and families about hospice philosophy and resources.
• Coordinate care with referral sources, providers, and community partners.
• Document assessments and services in EMR.
• Meet organizational performance expectations including a 75% admission conversion rate.
• Ensure compliance with regulations and policies.
