Job summary

SouthernCare in Ohio is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and referral processes, coordinate admissions, and support community awareness of hospice services. You will receive referrals from hospitals and health professionals, assess eligibility, educate patients and families about hospice care, and serve as a liaison between referral sources and the care team.

Qualifications

• Licensed RN with 3–5 years of hospice experience.
• BSN with 5+ years hospice experience preferred.

Responsibilities

• Receive and respond to referrals from hospitals and health professionals.
• Assess patient eligibility for hospice services.
• Educate patients and families about hospice care.
• Coordinate intake and admissions ensuring legal, clinical, and payer requirements are met.
• Liaise between referral sources and hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities and maintain accessibility with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify new outreach opportunities.
