Qualifikationen

• Licensed Nurse with three to five years of hospice experience
• BSN with five or more years of hospice experience preferred
• Minimum of three years of hospice experience required
• Prior experience as discharge planner, case manager, or educator preferred
• CPR certification required

Aufgaben

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other healthcare professionals
• Assess patient eligibility for hospice services by reviewing medical records and conducting clinical evaluations
• Educate patients and families on hospice care and processes with clarity and compassion
• Complete necessary documentation and coordinate intake/admission meeting legal, clinical, and payer requirements
• Serve as liaison between referral sources and hospice care team for seamless handoffs and rapid response
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners, case managers, and physicians
• Build and maintain relationships with referral partners and identify outreach opportunities
