Job summary

OhioHealth at Home in partnership with Compassus is seeking a Hospice Clinical Liaison (RN) to promote hospice care and collaborate with hospitals, facilities, and referral sources. You will assess patient needs, explain services, obtain consents, and coordinate admissions with the care team.

The role emphasizes building professional relationships, supporting growth targets, and ensuring accurate documentation throughout the admissions process.

Qualifications

• RN license required with nursing degree preferred.
• 2–3 years of related experience in healthcare settings.
• Hospital/long-term care experience is valued.
• Experience in healthcare marketing is a plus.

Responsibilities

• Attend staff meetings, trainings, and community events as needed.
• Explain hospice services and benefits to patients and families.
• Coordinate admissions with hospital, facility staff, and physicians.
• Obtain necessary signatures and ensure documentation is complete.
• Maintain professional relationships with community and referral sources.
• Support education efforts to hospitals and facilities about hospice care.
• Assist with the admission/discharge process and care coordination.
• Perform additional duties as assigned.
