Reserve Care, formerly Hospice of the Western Reserve, is seeking a Clinical Admissions Nurse (RN) for Regular Full-Time roles at East Campus in Willoughby Hills, OH. The role ensures programs are represented to patients and families through assessment, information sharing and admissions in home or facility settings, collaborating with the Referral Department.

Qualifications

• Graduate of an accredited nursing program: BSN and/or CHPN.
• Ohio RN licensure is required; valid driver's license with insured vehicle.
• Minimum of one year of nursing experience in intake, home care, hospice and/or oncology.
• Experience with volunteer management and HR practices preferred.

Responsabilités

• Provides information assessment and admission services to referred patients in home or health care facility settings.
• Communicates with the Referral Department, referring Physician, and Hospice MDs to ensure appropriateness of referrals and required certifications.
• Keeps the Referral Department informed about progress of referral/admission.
• Applies nursing principles to provide care, emotional support, and education to patients and families.
• Utilizes EMR and maintains timely, accurate documentation.
• Collaborates with the IDT to coordinate care and meet patient needs.
