Job summary

OhioHealth at Home in partnership with Compassus is seeking a Hospice Clinical Liaison to promote Compassus values and assist referral sources in aligning patient needs with hospice services. The role involves collaboration with hospital and facility education teams and admissions coordination to ensure smooth patient transitions.

Qualifications

• RN license required; Bachelor's in Nursing preferred.
• 2–3 years of related clinical experience.
• Healthcare marketing experience preferred.
• Hospital or long-term care clinical experience valued.
• Strong math, writing, and verbal communication skills.

Responsibilities

• Attends staff meetings, educational programs, and community events.
• Explains hospice services to patients, families, and others to facilitate accurate view of hospice.
• Coordinates admissions with Admission Coordinator and ensures forms are completed and submitted.
• Builds professional relationships with community, hospital, and facility education reps to drive growth.
• Manages transitions from hospital to home hospice and coordinates related services.
