Job summary

21st Century Home Health Services is seeking a Full Time Physical Therapist Case Manager, Level 2 for the Berkeley area. You will evaluate patients in their homes, develop care plans with PTAs, and provide skilled services to restore function and independence.

Expect collaboration with clinicians and physicians to ensure high-quality home health care. Requirements include a CA Physical Therapist license, BLS/CPR, a valid CA driver's license, and experience supervising PTAs is a plus.

Qualifications

• Current CA Physical Therapist License required.
• 1+ year experience supervising PTAs in HH environment a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.

Responsibilities

• Perform home evaluations, develop care plans with PTAs, and follow the plan.
• Assist patients with mobility and independence.
• Create detailed treatment plans for each patient.
• Coordinate with patients’ physicians to ensure quality care.
• Evaluate patient progress and report to MD.
