Job summary

Adaptive Home Health in Texas is seeking a Physical Therapist (PT) to join our field-based team. You will own the caseload, deliver skilled PT visits, and build a plan of care focused on gait, transfers, strengthening, and home safety.

This full-time role offers productivity-based compensation ($75.00 per point), with an expectation of ~30 points/week, and daily travel across the assigned territory. You’ll work with Homecare Homebase and coordinate with RN/MD, empowering patients and caregivers.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and independence in the field.
• Clear communication and thorough documentation.
• Reliable follow-through on orders and goals.
• Daily travel across the assigned territory.

Responsibilities

• Own the caseload as case manager for therapy plan and outcomes.
• Deliver skilled PT visits and in-home assessments.
• Build and execute a plan of care: gait/balance, transfers, mobility, home safety.
• Set goals, track progress, adjust treatment as needed.
• Coordinate with care team and supervise PTAs when appropriate.
• Coach patients/caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).
