Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to own a field-based caseload, delivering comprehensive in-home PT visits and building a clear plan of care for gait, transfers, strengthening, and safety. You will coordinate with RN/MD, document thoroughly, and supervise PTAs as needed.

You will work full-time Monday–Friday with a focus on reducing administrative burden and maximizing patient outcomes using cutting-edge charting and scheduling tools.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope: safety-first mobility, gait training, balance/fall prevention, transfers, strengthening, home safety assessment/modification.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Drive multidisciplinary coordination: communicate changes, align orders, move care team forward.
• Manage treatment and outcomes directly, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
