Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to manage a field-based caseload across Texas. You will perform in-home assessments, deliver skilled PT visits, and drive a clear plan of care focused on gait, balance, transfers, strengthening, and home safety.

You’ll coordinate with the care team, monitor progress, and adjust treatment as patients improve. The role includes PTA supervision where appropriate and emphasizes practical home programs for patients and caregivers.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and thorough documentation skills.
• Reliable daily travel across assigned territory with a valid driver’s license.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Develop and execute a clear plan of care: gait/balance, transfers, strengthening, mobility, safety.
• Set measurable goals, track progress, adjust treatment as needed.
• Coordinate with RN/MD and care team to align on orders and care.
• Supervise PTA where appropriate and coach patients/caregivers with home programs.
• Document promptly in Homecare Homebase (HCHB) with accuracy.
