Job summary

Adaptive Home Health, LLC seeks a Field-based Physical Therapist (PT) to deliver skilled home health services across Texas. The role focuses on caseload ownership, comprehensive in-home assessments, and outcome-oriented therapy plans.

The PT will work Monday–Friday with occasional weekends, using Homecare Homebase for documentation, and will operate under a productivity-based compensation model at $75 per productivity point.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope: safety-first mobility, gait training, balance, transfers, home safety.

Responsibilities

• Own the caseload as case manager: therapy plan, progression, and outcomes.
• Complete in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Drive multidisciplinary coordination and keep the care team aligned.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
