Job summary

Adaptive Home Health, LLC in Texas is seeking a Physical Therapist to join our field-based team. You will own caseloads, perform in-home assessments, and deliver skilled PT visits to patients in their homes, coordinating with the RN/clinical team as needed.

This role emphasizes a clear plan of care, measurable goals, and patient education. You will operate under a productivity-based pay model with about 30 points per week, supporting flexible schedules and travel across the assigned territory.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD with clear rationale.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and willingness to travel across assigned territory.
• Able to carry/transport basic therapy equipment.
• Competency in core home health PT scope: transfers, gait training, balance/fall prevention, and home safety.
• Strengthening, functional mobility, and home safety assessment/education.
• Patient/caregiver coaching with practical home programs.

Responsibilities

• Own the caseload as a case manager: accountable for the patient’s therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
• Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
• Manage treatment and outcomes directly, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs that actually get done.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
