Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to own the caseload with comprehensive in-home assessments and skilled visits. You will drive gait/balance, transfers, strengthening, and home safety, documenting in the patient’s chart and coordinating with the RN/MD for safe, effective care.

The role emphasizes field-based, patient-centered care with a productivity-based model and daily travel across the assigned territory. A Texas PT license and strong clinical judgment are required.

Qualifications

• Strong clinical judgment and comfort working independently in the field.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope with gait/balance and safety focus.
• Experience supervising PTAs and coordinating care plans is a plus.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, functional mobility, home safety.
• Set measurable goals, track progress, adjust treatment as needed.
• Drive multidisciplinary coordination: communicate changes, align on orders, keep care team moving.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
