Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to manage a home health caseload across Texas. This full-time, field-based role offers a productivity-based compensation plan at $65 per point (28 points per week).

You will perform in-home assessments, deliver skilled PT visits, and build care plans focusing on gait, transfers, strengthening and home safety. You’ll collaborate with RN/MD and document in HCHB.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and ability to work independently in the field.
• Comfort escalating changes in condition to RN/MD with clear rationale.
• Strong communication and documentation.
• Valid driver’s license and willingness to travel daily.
• Ability to carry/transport basic therapy equipment.
• Competency in core home health PT scope: gait/balance, transfers, strengthening, home safety.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression and outcomes.
• Complete in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, home safety.
• Set measurable goals and track progress; adjust treatment as needed.
• Drive multidisciplinary coordination with the care team.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).
