Adaptive Home Health is seeking a Physical Therapist (PT) to manage a field-based caseload in the Houston area. You will deliver in-home visits, develop plans of care, and collaborate with a multidisciplinary team to improve patient outcomes.

This full-time role offers a productivity-based pay model at $75 per point plus mileage reimbursement, with an anticipated 28 points per week and travel across the assigned territory.

Formación

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver\'s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope: gait/balance, transfers, strengthening, mobility.

Responsabilidades

• Own the caseload as a case manager: responsible for therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, track progress, adjust treatment as needed.
• Drive multidisciplinary coordination: communicate changes in condition, align on orders.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly.
