Job summary

Adaptive Home Health in Texas is seeking a Physical Therapist to provide skilled home health services on a full-time basis.

You will own the caseload, perform in-home assessments, and advance gait, transfers, strengthening, and safety while coordinating with the care team to ensure positive patient outcomes.

The role offers a productivity-based compensation at $75 per point with an expected 30 points per week, and the scheduling and field-based work support travel to keep you in the home.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Excellent communication and documentation skills.
• Reliable follow-through on orders, frequencies, and patient goals.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Drive multidisciplinary coordination and keep the care team aligned.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB.
