Job summary

Elevance Health is seeking an LTSS Service Coordinator – Clinician to support members across Davidson County, TN and surrounding areas. The role is field-based with periodic office attendance to participate in meetings and training, aligning with our hybrid work approach.

You will assist with assessments, care plan development, and coordination with RNs and the care team to meet member needs. The position requires licensure as LPN/LVN, LSW, LCSW, or LMSW or another RN-allowed license and at

Qualifications

Requires LPN/LVN, LSW, LCSW, LMSW or license other than RN in accordance with state law and nursing diploma or AS in Nursing.
Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.

Responsibilities

Assists in identifying members for high risk complications and gathers clinical data as directed.
Coordinates care for members with chronic illnesses in collaboration with RN and health care team.
Supports development and monitoring of care plans to meet member needs.
Participates in cost-effective and efficient utilization of health benefits.
Uses clinical guidelines to inform decision making while recognizing scope of nursing judgment.
