Qualifications

• Bachelor's degree in human behavioral sciences or related field (e.g., human services, nursing, social work, psychology, public health)
• Minimum 5 years of LTSS experience
• 1+ year MS Office (Word, Excel, Outlook)
• Reliable transportation and travel up to 50% within assigned territory to meet with members and providers

Responsibilities

• Intake/screening, assessment, development of support plans, ongoing case management and monitoring of member's health; documentation in prescribed system
• Respect confidentiality and maintain member privacy per handbook; uphold confidentiality as a critical component of the role
• Conduct monitoring contacts with members quarterly or as warranted by condition or program rules
• Conduct face-to-face contacts with members at least every six months and update care plans accordingly
• Complete new assessments during reassessments and as needed; conduct additional assessments as required
• Monitor service delivery and PAR; attend CMA training and agency in-service sessions
