Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician (Case Manager) to lead member care in the field across specified Ohio counties. The role focuses on assessment, care planning, and coordination of long-term services and supports with the health team.

The position requires an active Ohio RN license and at least 3 years in case management or service coordination for individuals with chronic conditions. Hybrid work and occasional office attendance are possible.

Qualifications

• Requires high school diploma or GED
• Minimum 3 years of experience with chronic illnesses/disabilities in a service coordination or case management role
• Current, active RN license in Ohio

Responsibilities

• Perform telephonic and in-person functional assessments to identify, evaluate, coordinate and manage member needs across physical health, behavioral health, social services, and long-term supports.
• Manage members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Obtain thorough member history to develop individualized care plans and set short/long-term goals with the member and care network.
• Develop and monitor care plans, facilitate authorizations/referrals, and coordinate with Medical Directors and interdisciplinary teams.
