Pacific Health Group is seeking a field-based Housing Stability Specialist to support CalAIM Community Supports. You will conduct assessments, coordinate housing resources, and provide person-centered case management to high-needs members across community settings.

The role emphasizes independence, strong time management, and collaboration with housing authorities, healthcare providers, and community organizations to ensure successful housing placements and ongoing tenancy support.

Qualifications

• Strong understanding of the housing system and local housing resources.
• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Strong understanding of healthcare systems, managed care, and community resources.
• Excellent communication, empathy, and cultural competence.
• Proficiency with technology, including case management systems and EHR.

Responsibilities

• Support housing transition navigation and tenancy services; coordinate housing deposits.
• Conduct intake assessments and develop housing-focused care plans.
• Build partnerships with housing providers, shelters, landlords, and community organizations.
• Travel locally within the county to support field-based members and visits.
• Maintain timely, accurate documentation of all member interactions.
