Job summary

Pacific Health Group is hiring for a field-based housing stability and care coordination role in Mendocino County, CA. You will support CalAIM Community Supports housing programs, perform intake assessments, build partnerships with housing providers, and guide members through the housing care continuum.

The position requires 3–5 years in case management or healthcare, independence, and strong time management. Travel within the county and surrounding areas is expected in this hybrid role.

Qualifications

• 3–5 years in case management, social services, or healthcare
• Strong understanding of Medi-Cal, CalAIM, and Community Supports programs (preferred)
• Excellent communication, empathy, and cultural competence
• Must document in real-time and manage member data accurately

Responsibilities

• Provide Housing Transition Navigation Services to support housing placements
• Coordinate Housing Tenancy & Sustaining Services and Housing Deposits
• Collaborate with housing authorities, landlords, shelters, and healthcare providers
• Deliver person-centered housing care management to high-needs members
• Maintain accurate, timely documentation of all interactions in real-time
