Job summary

The Center for Urban Community Services (CUCS) is seeking a dedicated Case Manager to provide person-centered services to a designated caseload. This role involves working both onsite and in the community to support housing stability, health, recovery, employment, and education goals.

Qualifications

• Bachelor's Degree or higher required for some levels.
• High School Diploma or equivalent required for Case Manager I or II.
• Relevant work experience may substitute college credits at HASA-funded sites.

Responsibilities

• Develop and maintain individualized service plans.
• Coordinate housing, healthcare, and social services.
• Assist with benefits applications and renewals.
• Provide crisis intervention and discharge planning.
• Deliver services onsite and in community settings.
• Maintain accurate case documentation in required records.
