ADP, Inc. in California is seeking a Service Coordinator I – Community Supports Generalist for the ILOS program. Under the supervision of the Program Manager, this role provides field-based care management and outreach to support Medi-Cal managed care members.

Responsibilities include housing navigation, deposits, tenancy and sustaining services, benefits advocacy, and collaboration with health plans to ensure high-quality, person-centered care.

Qualifikationen

• AA degree or equivalent.
• 2+ years of case management experience.
• A valid California driver’s license and clean driving record.
• Reliable vehicle and valid automobile insurance are mandatory.
• Clean DOJ background check.
• Bilingual English/Spanish preferred.

Aufgaben

• Engage with referred members in ILOS-CS to enroll and support them.
• Provide housing navigation, tenancy and sustaining services.
• Assist with housing deposits and securing housing.
• Advocate for benefits and assist with Social Security/SSI processes.
• Coordinate with health plans and community partners to ensure quality care.
