Job summary

Pacific Health Group seeks a Field-Based Housing Specialist to support Medi-Cal members with housing navigation, care coordination, and access to housing resources. You will conduct intake, assessments, and direct member support in community settings, with significant time spent in the field across Santa Cruz County and surrounding areas.

You will build partnerships with housing providers, shelters, landlords, and community organizations, ensuring timely housing placements and ongoing tenancy

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Strong understanding of housing systems and local housing resources.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Excellent communication, empathy, and cultural competence.

Responsibilities

• Provide housing stability services including Housing Transition Navigation and Housing Tenancy & Sustaining Services.
• Coordinate housing deposits and landlord engagements; assist with documentation and applications.
• Conduct field visits (65% time) in homes, shelters, and community settings; travel within county.
• Develop individualized care plans and coordinate healthcare and social services.
• Maintain thorough real-time documentation and ensure regulatory compliance.
