VitalCaring seeks a Hospice Registered Nurse (RN) to lead patient-centered care in homes, facilities, and community settings, delivering comfort and dignity.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead comprehensive, patient- and family-centered hospice care plans.
• Coordinate care with the interdisciplinary team and physicians.
• Educate and support patients and families throughout the journey.
• Maintain accurate documentation and participate in care planning.
