Job summary

VitalCaring, a field-based hospice provider, is seeking a Registered Nurse (RN) to lead compassionate, patient- and family-centered end-of-life care in patients' homes and community settings.

You will coordinate plans of care, manage symptoms, and collaborate with an interdisciplinary team to ensure dignity, comfort, and quality of life, while guiding families through difficult decisions with empathy and clinical excellence.

Qualifications

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care and coordinate comprehensive plans of care.
• Serve as the care coordinator to collaborate with the interdisciplinary team.
• Support patients and families with education, emotional support, and guidance.
• Drive quality and compliance through accurate documentation and care planning.
