Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered care in home and community settings. You will assess needs, develop comfort-focused plans, and provide symptom management with compassion and clinical excellence.

In this field-based role, you’ll coordinate with an interdisciplinary team, educate families, and advocate for patient wishes while maintaining high standards of quality and compliance in end-of-life care.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care: conduct comprehensive patient and family assessments; develop and manage individualized, comfort-focused care plans; deliver skilled nursing care with focus on pain and symptom management.
• Serve as the Care Coordinator: act as case manager, coordinating care across the interdisciplinary team; collaborate with physicians, social workers, chaplains, and aides; monitor patient condition and adjust care plans to maintain comfort.
• Support Patients & Families: provide education, emotional support, and guidance; help families navigate complex decisions with empathy and clarity; build meaningful, trusting relationships.
• Drive Quality & Compliance: maintain accurate, timely documentation; participate in IDT meetings and care planning; support quality and compliance standards.
