Job summary

VitalCaring Group is seeking a Hospice Registered Nurse (RN) to lead patient-centered hospice care in field settings. You will coordinate care, manage symptoms, and advocate for patients and families with empathy and clinical excellence.

You will work autonomously within an interdisciplinary team to ensure comfort, dignity, and alignment with patient goals, while maintaining documentation and quality standards across visits.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Lead Patient-Centered Hospice Care: conduct assessments, develop and manage care plans, manage pain and symptoms
• Serve as the Care Coordinator: act as case manager, coordinate care with interdisciplinary team, monitor and adjust plans
• Support Patients & Families: provide education, emotional support, guidance, and assist with difficult decisions
• Drive Quality & Compliance: maintain timely documentation, participate in IDT meetings, uphold standards
