VitalCaring is seeking a Hospice Registered Nurse (RN) to lead field-based, patient-centered care focused on comfort and dignity at end-of-life. You will manage plans, assess needs, and work closely with families in homes and facilities.

You’ll coordinate care with clinicians, social workers, chaplains, and aides, ensuring compassionate, goal-aligned outcomes. A strong clinical background in hospice or similar settings is essential.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation
• Experience in hospice, palliative care, or home health
• Case management and interdisciplinary care experience
• Ability to quickly build trust with patients and families
• Experience with Homecare Homebase (HCHB)

Responsibilities

• Lead patient-centered hospice care with individualized care plans
• Serve as care coordinator across the interdisciplinary team
• Support patients and families with education and guidance
• Drive quality and compliance through accurate documentation and care planning
