Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two years of healthcare or related sales experience.
• Strong communication, relationship-building, and influencing skills.
• Ability to manage multiple priorities in a fast-paced, field-based environment.
• Proficiency with Microsoft Office and CRM systems.
• Demonstrated ability to work independently and drive results.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured outreach and relationship-building with referral partners to grow referrals.
• Collaborate with hospitals, SNFs, and physician offices to support timely admissions.
• Track activity and outcomes in CRM and EMR systems to monitor performance.
• Meet or exceed established referral growth and admission goals.
