Qualifications

• High School Diploma/GED (or higher) AND 5+ years with ABD/SSI in clinical health care
• Associate's degree with 3+ years of clinical experience
• Unrestricted LPN/LVN license in TX with 3+ years of clinical experience
• CHW certification with 3+ years of clinical experience
• Intermediate proficiency with MS Office (Word, Excel, Outlook)
• Access to reliable transportation and travel up to 75% of time

Responsibilities

• Complete face-to-face assessments in members' homes.
• Travel up to 75% of the time, up to 100 miles from duties.
• Meet documentation/turnaround times per state contracts.
• Time management and organizational skills.
• Create, edit, save and send documents, spreadsheets and emails.
• Reside within commutable distance of assigned duties.
• Work on Service Plan
