Qualifications

• Reliable transportation, a valid driver’s license, and ability to travel within assigned territory.
• High-speed internet at residence.

Responsibilities

• Assess, plan, and implement person-centered strategies.
• Connect members with financial, psychosocial, community, and state resources.
• Coordinate care across the continuum and serve as a key point of contact.
• Advocate for members and families to ensure their needs and preferences are heard.
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
• Travel 50% of the time within the Bexar County, TX area and surrounding areas.
