Job summary

Qualifications

• Must have one of the listed qualifications with clinical experience.
• Proficiency with Microsoft Office suite.
• Access to reliable transportation and ability to travel up to 75% of the time.

Responsibilities

• Complete face-to-face assessments in members' homes.
• Travel up to 75% of the time (up to 100 miles from location).
• Meet documentation and turnaround times per state requirements.
• Coordinate members' services and care plans.
• Create, edit, save, and send documents, spreadsheets, and emails.
• Reside within commutable distance of assigned duties.
